Jerry Ray Herden Jr., 58, of Lebanon, died Sunday, Nov. 20, 2021, in Lebanon.
He was born Oct. 21, 1963, in Phoenix, Arizona.
Arrangements have been entrusted to Midwest Cremation and Funeral Services, of Springfield.
