Gerald Dale “Jerry’’ Cook, 64, of Phillipsburg was born April 19, 1959, in Lebanon to Robert A. and Fern I. Woods Cook and died Wednesday, July 5, 2023, in Springfield. He is survived by one son, Dale Cook of Phillipsburg; one daughter, Iona Reed and her partner Cody Sterling of Taylorsville, Utah; two grandchildren and twin baby boys on the way; three brothers, James “Jim’’ Cook and his wife Helen, Jack Cook and his wife Karen and Terry Cook and his wife Donna, all of Lebanon; two sisters, Peggy Bohannon and her husband Elvin of Conway and Barbara Hicks of Springfield. Arrangements are incomplete and will be announced by Shadel’s Colonial Chapel. At his request, his body is to be cremated with burial at a later date at Mt. Zion Cemetery.
